The San Diego Integration Institute is a Program of Health Quality Partners of Southern California (HQP).

  • HQP has a contract with County of San Diego, HHSA, BHS to implement the Integration Institute, which began on May 1, 2011.
  • The overall purpose is to develop, implement, and evaluate an Integration Institute to promote the vision of shared population management of behavioral health and physical health illness through working relationships of paired behavioral health (BH) and primary care (PC) organizations.
  • The goal of the Institute is to support BH and PC providers to enhance intra-organization integration and in achieving integration between PC and BH service organizations, including alcohol and other drugs services.
  • The Integration Institute encourages organizations and providers to lead change at the provider, program, and system levels in order to reduce barriers and increase integration.


FUNDING 

Funding is provided by the Mental Health Services Act (MHSA) and the County of San Diego HHSA, Behavioral Health Services through a Contract, which was granted to the  Health Quality Partners, a subsidiary of the Health Center Partners.
